This is an excellent online course evaluation resource. The various assessment forms, including the self-assessment ones are especially useful. All materials are made available for use through the Creative Commons License. Well done CSU and thanks for sharing!

Thank you for sharing! We are proud of our product and are happy to share. The site is going through a redesign (completion fall '11). The a new look will have greater usability and an interactive self assessment form and (potentially) links to exemplary examples.
